As practicing surgeons, most of us will be keenly aware of the fact that success in orthopedic trauma surgery, and ultimately good patient outcomes, depends on sound, evidence-based scientific research as much as it does on the skill and expertise of the attending physician. However, in our daily work, we regularly treat patients who present with highly individualized and exceedingly complex injuries—especially in the field of upper-extremity trauma.
Compartment syndrome, neurological impairment, or post-traumatic stiffness are typical early difficulties in this context, while non-union, malunion, or post-traumatic arthritis are among the complications that manifest later and frequently require additional surgery.
Conditions such as severe articular comminution, where the bones in a joint have shattered into numerous small pieces due to a high-energy impact, also frequently lead to complications. Other injuries may require meticulous surgical release, or complex reconstruction using structural grafts.
When fundamental treatment algorithms are not enough
Unfortunately, in many of these cases, the available scientific evidence on which we can base our treatment decisions is not very strong. The reason for this is self-evident: high-level randomized controlled trials with sufficient sample sizes are difficult to conduct for highly specific trauma scenarios involving unforeseen complications.

Meticulous pre-operative planning, detailed anatomical knowledge, as well as an intimate understanding of the associated biomechanics are crucial for navigating distorted anatomy resulting from unusual injuries.
Professional experience also becomes much more relevant in these types of situations: the higher the level of complexity, the more dependent treatment success becomes on the experience of the attending surgeon.
